PROVIDER REFERRALS

1. We are currently only accepting referrals for patients under the age of 18 years of age for most testing at this time.

   We do consider scheduling neuropsychological testing for 18-25 year olds on a case-by-case basis and are happy to provide referrals for other adult providers if we are not a great fit.

   Please do NOT send referrals for dementia evaluations-thank you!

2. Please email or fax over a referral order for neuropsychological testing to admin@nashvilleneuropsychology.com or fax to 844-270-1951. Include the Diagnostic code (ICD-10 or DSM-5) being used for the evaluation to document medical necessity for the evaluation if patient has BCBS, Aetna, or UnitedHealthcare.

   
   *Note: We can only bill under the Medical side of insurance plans NOT under their Behavioral Health plan (typically F codes), so please have the provider use the appropriate medical code when submitting the referral.

3. Have the patient’s parent/guardian or patient call our office at 615-212-9875 to complete a phone intake or direct them to our contact page on our website to complete the form online.
